Licensed Practical Nurse Support Manager

Our client provides housing, healthcare, meaningful employment, and recreational activities to individuals with special needs throughout New Jersey.

Seeking an LPN Support Manager to use best practices and technology within a community based residential setting to teach people with developmental disabilities how to acquire the physical, intellectual, emotional and social skills needed to live as independently as possible. Additionally, to provide nursing and direct care services for individuals with disabilities living in the home. Will address any issues regarding the health, safety, and/or welfare of the above-mentioned individuals.

REQUIREMENTS:

· Graduate of accredited Nursing School; LPN

· Licensed in the State of New Jersey

· One year of experience working with individuals with intellectual and/or developmental disabilities (I/DD) is required; three years of experience preferred

· Valid Unrestricted Driver’s License in good standing

· Ability to lift and transfer at least 50 pounds

· The ability to sit, bend, squat, reach, and stand for several hours at a time

· Basic Computer Literacy

· Must be able to read, write and verbally communicate in English and direct/respond to individual’s needs

· The ability to safely and effectively execute all Personal Control and Defensive Techniques

· The ability to exercise good judgment and use common sense.

· The ability to remain calm in crisis situations.

· The ability to interact with a wide range of people with respect.

· The ability to approach daily work from a person-centered vantage point.

RESPONSIBILITIES INCLUDE:

1. A 40-hour salaried, unlimited schedule, to include at least 8 hours of evening work per week and direct care.

2. “On call’ 24 hours a day in cooperation with other management staff.

3. Objectively completes staff evaluations.

4. Monitors and maintains residential budget.

5. Monitors and schedules required staff training, as well as relevant seminars, conferences and additional training.

6. Monitors and insures accurate “in house” documentation.

7. Participates in Circle(s) of Support for personal futures planning.

8. Reports, monitors and schedules house and vehicle maintenance.

9. Assists in maintaining and monitoring of individual finances, as needed.

10. Builds a support network between the individuals and community members.

11. Documents person’s progress for support people including family, staff, employment specialist, case manager/support coordinator and other professionals as necessary.

12. Ensures consistent service delivery, which connects work, home, and recreational needs.

13. Serves as an active member of each person’s interdisciplinary team, to include involvement in Individual Habilitation Plan (IHP)/Individualized Service Plan (ISP) development.

14. Accurately completes all in-house documentation including but not limited to Vehicle Log, behavioral tracking, Antecedent Behavior Consequence (ABC) forms, Unusual Incident Reports (UIRs), Critical Information Log, Medication Administration Reports (MAR), Progress notes and Monthly reports as assigned.

15. Submit monthly written reports covering operational status, as well as individual progress and concerns to supervisor.

16. Attend conferences, seminars, and training relevant to the position

17. Provide direct nursing care effectively by ensuring the general well-being of each

Individual served while promoting a consumer-oriented approach to health care.

18. Implements medically prescribed diets and exercise programs.

19. Assists Community Support Nurse in training of Support Staff in the areas of

health maintenance, medications, and medical treatments.

20. Works with each individual’s Interdisciplinary Team (IDT) and all other interested

parties to build self-esteem and social capital for each individual served.

21. Successfully completes required residential training, including: CPR and First Aid; Medication Administration; Preventing Abuse, Neglect and Exploitation; Shifting Expectations, Life Experience and Services; Life Threatening Emergencies (Danielle’s Law); New Hire Orientation; Crisis Management; Corporate Compliance; General Behavior; In-House Orientation; and any required specialized trainings.

22. Successfully and consistently scores a 90% rating or higher on Quality Assurance audits. A rating of 89% or less for 3 consecutive reviews may result in immediate termination.
